Nurse- Family Partnership (NFP) is an evidence- based, community health program that serves women of low-income pregnant with their first child. Each mom is partnered with a registered nurse early in her pregnancy and receives ongoing nurse home visits until the child turns 2 years of age. During visits, nurse home visitors will educate on personal and environmental health (both during pregnancy and after delivery), infant/ toddler health and development, and parenting skills. Mothers will also receive support and education on meeting life goals, and will be referred to community agencies, as needed. Home visits will be made weekly, bi- weekly, or monthly.

Eligibility Requirements

Our program serves women and teens who are pregnant for the first time. To be eligible a client/family must be at or below the 200% poverty level. Participant must be less than 28 weeks, pregnant to enroll in the program.
